Question 1: Description of the person chosen: A comprehensive description of the person, their impairment, and their needs - physical, emotional, social, intellectual and spiritual needs. Outlining the needs that are being met and those which are not.

Question 2: Describe the level of assistance required and in what areas. Accurate identification of the level of assistance required by the person - consider assistance requirements, mechanical aids which may help. Does the person have social and communication needs? Are there health and safety issues? Document your findings (Refer to Roper Logan and Tierney, Activities of Daily Living).

Question 3: Addressing the needs of the person which are not being met. What action can be taken to address the needs not being met? This should include an awareness and sensitivity to the needs of the person and should include the person's choice and involvement. It should also refer to the following: - Show clearly how you intend to improve quality of life for the person while demonstrating techniques to ensure following. Privacy & Dignity, Independence, Positive Self-Image, Confidentiality. Clear action plan to meet the person's needs

Question 4: You should also draw on other learning experiences you have had. This may be additional class-based learning or through work experience or additional reading. Include evidence of reflection on the experience of working with the person described

Sample Case Study Michael Murphy is a 76 year old man. He suffers from arthritis in his hands and knees, high blood pressure, anxiety and has a history of a minor stroke 6 years ago. He hasn't seen his GP in 2 years and gets his prescription from the receptionist in the surgery every 3 months. He has been on the same medications for 2 years. He often gets confused about which medications he needs to take and at which time. He lives alone in a house that he rents from the local county council in a large estate in the town. The estate sees a lot of anti-social behaviour and this frightens Michael. His wife died 6 years ago. He cries whenever she is discussed in conversations. He does not have any other family living near him. His two sons are married with children and both live a significant distance away. His house is two storied with an upstairs bathroom. It has a large garden which is becoming overgrown and inaccessible to Michael. He has lost a lot of weight in the last 4 years and may now be underweight. He eats tea and toast mostly as he cannot cook. Michael does not have any aids or appliances to help him in his home
