Background (History of present complaint)	This is a 25-year-old male admitted 3 days ago with alcohol intoxication. The patient required intubation and sedatives during acute illness. He was extubated yesterday and is medically stable. The Psychiatrist saw the patient this AM and has cleared the patient from being "a risk to himself". The social worker, hospitalist, and psychiatrist have all spoken to the patient about his extensive drinking and all are recommending the patient admit himself to an alcohol treatment center. The patient does not feel his drinking is a problem and is refusing this option

Patient Information (History, allergies, PCP)	Patient is a healthy 25-year-old male with no medical history. He is a 1 pack per day smoker and has been admitted on 2 previous occasions with alcohol intoxication. The patient has NKDA. His PCP is Dr. Smith, but has not seen this Physician in 6 years.

Additional information:

Yesterday during patient rounds, Mrs. Easter, the patient's mother, becomes irate after hearing her son, the patient, will be discharged to home in the morning. The mother has expressed concerns that her son may go too far next time and drink himself to death. She voices frustration that no one in this hospital is doing anything for her son and feels it is because he is unemployed and has no health insurance. The mother is refusing to pick up her son if he is discharged stating he needs help with his drinking problem. The son tells his mother he will walk home and does not have a drinking problem; it just got out of hand a few times. The social worker has given the patient information on several alcohol treatment centers. Since the patient has no insurance, the cost is close to $6000/day and since the patient has not been declared a threat to himself, he can sign himself out AMA at any time.

Attitudes vary between interdisciplinary teams members with some stating phrases such as: " this patient will never change" and " this is a lost cause", while others feel it is their responsibility to fix the patient and his problems.
