Question A Based on the findings from the report (case study), describe the safety climate of the 455th Air Expeditionary Wing at Bagram Airfield, Afghanistan, in particular, the attitude towards safety displayed among the airmen. Support your analysis with the evidence reported. The role of factors, in a crash involving a transport plane. C-130J, Jalalabad Airfield, Afghanistan On October 2nd, 2015, around 12;16 AM local time, a C 130J aircraft with the tail number 08 3174 crashed after taking off from Runway 31 at Jalalabad Airfield (JAF). This incident occurred during the leg of a planned contingency airlift mission. The aircraft involved in the mishap was assigned to the 455th Air Expeditionary Wing based at Bagram Airfield in Afghanistan. The crew on board consisted of the pilot, co-pilot and two loadmasters from the 774th Expeditionary Airlift Squadron. Additionally there were two flies away security team members and five contractors who were traveling as passengers. Unfortunately all eleven individuals on board the aircraft and three Afghan Special Reaction Force members manning a guard tower lost their lives upon impact. The mishap resulted in damage to both the aircraft and its cargo load well as a perimeter wall and guard tower. The total value of the aircraft and cargo amounted to $58,363,044. Prior to this incident on October 1st, 2015, around 11;13 PM time (L) the same aircraft had landed at JAF after completing its leg of a contingency airlift mission. While the aircraft was, on the ground the military police officer placed a hard-shell case for night vision goggles (NVGs) in front of the control wheel during the process of loading and unloading with engines running. This was done to keep the elevator of the aircraft in a position to accommodate cargo. However neither the military police officer nor the main cockpit pilot removed the case during the 50 minutes before take-off at 0015L. During take-off with the cockpit pilot in control there was a rotation and lift off occurred at a speed three knots below what was expected. The pitch angle of the aircraft continued to increase due to obstruction caused by the shell NVG case blocking movement of flight controls. As a result it prevented the cockpit pilot from pushing on the control wheel to decrease pitch angle. The main cockpit pilot misidentified this problem with flight controls as a trim malfunction leading both pilots involved in this incident to apply recovery techniques. The rapid increase in pitch angle eventually resulted in stalling that couldn't be recovered by either pilot. 28 seconds after lift-off right of runway, within JAFs jurisdiction is where impact occurred.
