I also found it surprising how as the economic growth of countries continued, the fertility rate decreased with it. I also believe his message was simply that you cannot buy healthcare and not spend a lot of money. In order to see an increase in lifespan, you need to spend money to achieve that goal in lifespan, especially for developing countries.
