  1. The person's or organization's (the plaintiff's) name.
  2. An identification of what their copyrighted item is. Art? Music? Text? etc.
  3. Who they are suing, in other words, the name of the defendant.
  4. What do they want? Do they want the defendant to stop doing something, start doing something, pay some money, something else? If they want them to start/stop, what is the specific thing that needs to start/stop?
  5. A link to the principal source for your information. A URL that works.
