CASE STUDY: Jodi is a 22-year-old Hispanic female who has been addicted to heroin for five years. While on a binge three years ago, Jodi was raped and became pregnant. She gave birth to her daughter Natalie three months prematurely. Natalie is 2 and is currently involved in the foster care system because she was born addicted to heroin and there were no family members available to care for her. She lives with a foster family that has four other children. Jodi is able to visit her daughter weekly; however, for the past year she has visited only three times because she has been in active addiction. Jodi uses heroin five to six times per day intravenously. She has attempted recovery twice. Jodi attempted an outpatient detoxification program but did not complete the treatment. Jodi then decided to try an inpatient detoxification program, but she left treatment on the second day. Next, she began suboxone treatment but discontinued after one month. Jodi then returned to using heroin and has been actively using for the past nine months. She is currently using heroin daily, is working as a prostitute to supply her habit, and is homeless. Jodi has not been honest with her counselors about where she sleeps at night. Jodi has no contact with her parents. She states that her mother is currently in active addiction using crack cocaine and can't take care of Jodi's 15-yearold sister and 13-year-old brother. Jodi states that her father lives in another town but has had some problems with addiction in his past and is clean and sober now. She states that he went to a halfway house and has never returned to their hometown and has not corresponded with her since she was 15. Jodi states that she was so sad that her father would not contact her that she began smoking marijuana daily at the age of 15 and then began using alcohol at age 17. She reports that she graduated to heroin at the age of 17 with a friend out of curiosity and became dependent very quickly. Jodi received her GED at age 20 and had started a Certified Nurse Assistant training program at her local educational opportunity center, but did not finish because of substance use. She explained that she had time and attendance issues because of experiencing withdrawal. Jodi has been arrested for prostitution in the past. She is not on parole or probation and has never been involved with drug court, but states that she fears the legal system. Jodi says that she feels depressed all of the time because she misses her daughter and her siblings. When she is not using heroin, she misses the drug as well as her friends. Jodi says that recently she attempted suicide by taking pills she purchased from a friend. Jodi states that her friends called the ambulance and she was taken to the hospital. She is very angry with her friends for saving her life. She does not foresee recovery in her future and feels that she cannot make changes fast enough to obtain custody of her daughter and assist her siblings. Jodi had been diagnosed with posttraumatic stress disorder (PTSD) because of being raped three years ago. Jodi has never been officially treated for the PTSD in a group setting or individually. Jodi has had past employment as a waitress and a telemarketer. She reports that she does not keep a job for a long period of time because of the substance use. Her long-term goal is to be employed as a certified nurse assistant. Jodi has not seen a physician for a physical since she was in an inpatient facility. At the time of her medical examination she was underweight and had irregular menses. Jodi has been treated in the past month at the emergency room for an abscess on her left forearm. The abscess was packed and an antibiotic was prescribed. Jodi stated that she did not take the full prescription and has noticed some redness and swelling. Jodi's support system consists of her friends who are actively using. She has not attended a Narcotics Anonymous group since she was at the inpatient detoxification facility for two days. Jodi has been referred to counseling from the Department of Family and Youth Services because her daughter is in foster care and she has agreed to attend counseling sessions biweekly. Jodi states that she would like to begin a relationship with her daughter, and to develop better coping strategies when dealing with negative emotions.
