You have now had three sessions with Elizabeth "Liz" Smith. As a reminder, Liz recently had her 33rd birthday. She is a single, Caucasian heterosexual cis-gendered female. She came for counseling at the recommendation of a friend.
When she came to therapy, she presented with feelings of sadness, worthlessness, and shame due to her boyfriend's infidelity and status as a single woman. She has not had many romantic relationships, and this was her first long-term committed one. He cheated on her but she stayed for 3 years anyway. She identifies as Christian, and her ex-boyfriend is also the only person that she has had a sexual relationship. Her parents are married, and you learn in the clinical interview that there was some domestic violence in the home. She coped with it by being a high achieving student and not creating additional stressors for her family. There is a family history of depression and her father committed suicide five years ago.
Liz has worked at the same company for seven years. She is dissatisfied with her current job and feeling overall unhappy. She wants a new job but is reluctant to apply for a promotion or look for new positions. At work, her coworkers have noticed that she is often irritable and withdrawn, which is quite different from her typically upbeat and friendly disposition. She had called out sick, spending time at home watching TV or sleeping. Her friends and family were concerned because she was isolating and tearful. Your clinical interview revealed that she has frequent bouts of tearfulness, difficulty sleeping, trouble concentrating at work, decreased energy and decreased interest in pleasurable activities. She is also quite self-critical and often makes self-deprecating comments. In sessions she has been open, talkative and appears committed to therapy.
