"Liz Smith has now been in counseling with you for more than a year. She is a single, Caucasian heterosexual cis-gendered female. As a reminder, Liz came to see you around her 33rd birthday. She came for counseling at the recommendation of a friend. At that time, she presented with symptoms of depression which included difficulty sleeping, isolation and decreased energy and motivation. During the intake, she disclosed experiencing intense feelings of sadness, worthlessness and shame due to her boyfriend's infidelity and her status as a single woman. She also had started drinking more than usual and her friends and family were worried that she was abusing alcohol. She also reported being very irritable and cranky and difficulty with sustaining friendships. While she had never made a suicide attempt, she did admit to wishing at times that she was not alive. Your treatment plan addressed her symptoms of depressions and interpersonal relationships. Through the course of treatment, she started working with a psychiatrist and had a positive response to medication. She started taking a guitar class and reports being happier with her life. Her family relationships are stable and she has a few friends that she regularly talks to and chats with on social media. Since her time in therapy, her depressive symptoms have decreased significantly. She found a new job and started dating again. Several months ago, she started dating someone seriously and they have talked about the future. She is actually considering moving in with her new boyfriend.
