-Provide the reason for falling out of status and the exact timeline for when student was terminated.

-How will being reinstated benefit education goals as an F-1?

-How would the student demonstrate becoming a successful F-1 student if reinstated?

-What are some academic goals the student has to avoid falling out of status again?

Include the start date and end date of previous school.

-If OPT is applicable, include start date and when you fell out of status.

-If there is a gap between falling out of status to this present time, explain what has been done during this time and why no earlier action was done.

-If a student has been out of status for more than 5 months, will need to provide a reason as to why there was no action taken until now and what they were doing during that time
