Reiterate the problem and the root cause you are going to try to tackle for this assignment. Incorporate any feedback you received on assignment 3.
- Brainstorm possible changes you can make to address the root cause. If possible, include others in the brainstorming. If a personal problem, you might brainstorm with your spouse or child, if they are involved If at work, involve other members of your team who are impacted by the problem. This should only take a half-hour or less. Then, decide which change you are going to take on to test. The change should be one that will be reasonable to trial over one week and possibly see results. In your assignment submission, document the list of possible ideas that were drafted, and why the selected change was chosen.
- Identify at least one indicator you can use to measure whether the change creates any improvement. What this means is something you can measure. [e.g.: number of times you arrive to work on time, % of times you forget your keys,% of meeting invitees who showed up to the weekly huddle].
- Describe the indicator[s] you will use.
- Be clear about how you will measure and how you will be collecting your data along the way. If baseline data already exists, document it.
- Indicate how frequently will you measure this data?
- Identify your target for this cycle? [i.e.: what you hope to achieve in terms of the data tip: your measurement target should also be SMART].
- Using the PDSA worksheet found, test the change for one week and document for each step what you did and what the results were.
- Based on the results of the PDSA. identify your decision regarding the success of your change. Will you adopt the change, adapt the change, or abandon the change?
- Explain your thinking: why will you adopt, adapt or abandon the change?
- If you are going to adapt [change the intervention] what will you do differently?
- Reflect on and describe how the PDSA process went and any lessons learned for next time.
