Reply to this post. The coaches evaluation form that I received is the current Middle School coaches evaluation form, made by the Columbia Public Schools District. It is 3 pages long and asks for a score and comments for the following statements: positive coaching, communication, knowledge of the sport and activity, professional development, and administrative duties. You score based on a 1-5 scale. 1 being does not meet expectations, 3 being meets expectations, and 5 being excels expectations. You are allowed to score a 2 and 4 as well if you feel needed. Each expectation has a description of what it entails and how each expectation should look. These expectations are the values of Columbia Public Schools athletics and activities programs. This evaluation form will be filled out by the schools Athletic and Activities Director. The evaluation only has 5 statements of expectations to score, it also has the option to fill out comments for each section. I would like to see a few open ended questions on the evaluation as well as more values to score. I think the 5 they have are great and necessary but I fear they do not cover all that could be covered on a coaches evaluation form. I would also like to see an evaluation form that students can fill out after their season. I think that would provide important information and could help get a better understanding about how the coach's performance is
