Requirements: 1) Draw an REA diagram based upon the information provided below for the La Crosse Animal Hospital. 2) Use the database for La Crosse Animal Hospital that is based upon your own REA diagram. Create the database using Microsoft Access. Insert two records for each table. Please create your own data for the two records. La Crosse Animal Hospital You have been hired to develop an information system for the La Crosse Animal Hospital, which is owned by Dr. Rivers. Dr. Rivers currently owns a personal computer but uses the computer only for word processing. Dr. Rivers wants to use the system you have been hired to develop to maintain pet histories and accounting information. He has high hopes and is counting on you to help him through this change. After Dr. Rivers agreed to your development plan, he provided the following overview of his operation. When a new customer comes to La Crosse Animal Hospital, the parents of the pets are required to complete an introductory form that includes the following: Parent name Address Day phone Evening phone The parents are also required to provide the following information about each pet (Note: some people own many pets!): Pet name Breed Color Birth date The introductory information is to be stored so parents do not have to provide the same information each time a pet has an appointment. The doctor sees only one pet during each appointment. For example, if Dr. Rivers is going to see Emilys two cats, two appointments are established. For each appointment, the pets weight, the reason for the appointment, and the doctors diagnosis is recorded. Depending on the diagnosis, the doctor may prescribe any number of medicines to cure the pet. Parents are charged $25 for each appointment and must pay additionally for medicines.
During your interviews, the following attributes were also identified as being necessary for the system: Number of pets owned by each parent Price of each specific medicine prescribed Total charge for the appointment Quantity of each specific medicine prescribed Prescription price Medication list price Medicine name Total cash receipt amount Appointment date Cash receipt amount applied to a specific appointment Appointment length
