B. What’s the Mission Statement of the agency?

C. What are the funding sources for the agency?

II. Services/Programs Provided

A. What client services or programs are provided by this agency?

B. What clientele or population is serviced? i.e. older adults, teens, women, etc.

C. Where does this agency get its referrals from?

D. What type of record-keeping is done at the agency? How do they protect client confidentiality?

III. Vocational Positions

A. What are the position titles at the agency?

B. What are the qualifications and skills required or preferred for each position title? (education, experience, etc.)

IV. Work Environment

A. What are the work activities at the agency? In other words, what would a typical day be like?

B. What does the facility look like inside and outside?
