She was a product of normal birth with met all developmental milestones. Needed to repeat the 3rd grade for reasons of immaturity. Graduated from high school with grades in the C range. Took a leave from school and moved in with her parents last year. Both parents work (father-engineer, mother-accountant). Rosa speaks about her mother stating, "my mother is nice, but she has no power."When asked about her father she shook her head and did not respond. She states she has few friends and has never had a best friend in whom she could trust or confide. Her roommates confirm that since coming to college, she has shown little interest in developing friendships with them or with other women on their hall. She is often seen walking alone and appears to be mumbling to herself. She has some eccentricities, such as lining up all her personal effects on her dresser in the shape of the letter "M," not getting into a car without walking around it twice, and seemingly being obsessed with the island Greenland; she has flags, pictures, and all things Greenland decorating her room.

Since returning to school this semester, Rosa has had increasing difficulty with her studies. She is failing three of her four classes. In the last month, she has stayed mainly in her room, even skipping meals, and always has the blinds closed. When a roommate entered her room to open the blinds to let light in, to buck up Rosa's mood, Rosa threw a tantrum and quickly closed the blinds, screaming at her, "Keep these closed! Do you want them to find me? They want to take over all of us, don't you know?" Her roommates report that she keeps a light on all night and that whenever they wake up in the night, they hear her talking in her room, suggesting she is always awake when they check. They often find her walking in an oval pattern in her room, day, or night, always in the same path, which has begun to wear a pattern in the rug.

In the previous week, Rosa appeared more anxious, fearful, edgy, and irritable, and she complained about her roommates leaving the door and windows unlocked when they leave. This morning, two of her roommates came home from class to find her curled up on the floor and mumbling incoherently. When they asked what was wrong and if they could help, she began sobbing and mumbling. At that point, her roommates brought her to the Counseling Center where you work.

On Mental Status Exam, Rosa exhibited poor eye contact and responded to questions sporadically. She denied taking any drugs or alcohol in recent weeks. She denied seeing visions but would not answer when asked if she was hearing voices. Her mood was somewhat labile - anxious or dysphoric at times and flat and withdrawn at other times. When asked specifically what was going on, she responded, "My head is just so full; I just can't take it anymore. They won't leave me alone." She denied thoughts of harming herself or anyone else because it was against her religious beliefs.
