The worst aviation accident in history occurred on March 27,1977in Tenerifewhen two Boeing 747s collidedon a single runway. Several documents related to the accident are attached, and you can search the web to find additional information about the accident. One good source of information is:

Read a summary of the accident and the CVR transcript (several documents are attached in the instructional materials tab. Pick out several causes of the accident and discuss them in your initial post. Comment on your classmate's discussion posts and substantiate your comments based upon your opinion and the facts that you gleaned from your research.

We will only have this single thread this week, but I hope that it is instructive, because it will show aspects of airport operations, flight deck operations, communications, and what happens when one or more of the links in the safety chain fail.
