Briefly explain what prohibited your ability to be successful?
(-Be self reflective, most suspensions take multiple terms, so a single outside events rarely causes everything. Speak to what the events effected (time management, motivation, focus, etc..)
(-Did you reach out to your professors and advisor? why not?
(-Why did you not choose to take a break? This section is about taking ownership of past behaviors and learning from them in order to be able to be successful moving forward.

What has changed in your life that will now allow you to be successful?
(Describe how the circumstances have changed that will allow you to perform successfully. Include changes you have identified as beneficial in any of the following areas: academics, family, social, work, etc..) make sure everything that was mentioned in part one is resolved. work issues if you working less hours etc.. Address what you have done to improve the deeper issue (how have you improved your time management, etc.. Speak to the lessons you have learned.
Section 3:
Describe the specific plan of action that will follow to attain academic success if your appeal is approved. What success strategies will you follow?
(-This need to be all tangible tings. address plans to retake previously failed courses. what does your study space look like? Is it private? do you have desk? How do you manage your normal schedule? A calendar? Phone reminders? A whiteboard? Those work for course work as well. Did you plan to use use online tutor etc..

What is your action plan in the event your previous issues come up again?
(-please be specific. Who is your support system at home? Will they hold you accountable to your education? What will you do if the same thing happen again? What if your computer breaks? Do have a backup? Do you have one available at a friend's, or the library. what if you lose internet? Where are the 3 closest local places with free wi-fi?
