Paul is a 33 year old male with an intellectual disability. He has some skills with his ADL's, for example he is able to dress himself and attend to most of his personal hygiene needs. Paul is continent. His verbal communication skills are almost non-existent. He is now working in a sheltered workshop environment. Paul has only been in this environment for two days.

Day 1

Paul threw a hammer, narrowly missing one of the instructors. He also dropped a wood working saw on his foot, sustaining a mild injury.

Day 2

The workshop instructors are very concerned about Paul's self-harm practices. He is pulling his hair out, (little left in place now), and slapping his face, sustaining mild bruising and a skin tear from his finger nails. His finger nails have now been cut to a safe length.
