Select a hospital/healthcare organization of choice, then find its most recent Community Health

Needs Assessment. Does it meet the intent of the regulations? Does it identify population health needs

and address them in an affordable manner (e.g., strategy, planning, etc.)?

1. Introduction; information about the facility selected; how you accessed the information

2. Defining the Community Served (target populations, assessing needs, and community

input into the effort)

3. How well did the documentation aspect occur (data, methods, criteria)?

4. How did the findings compare to the facility's resources to address the significant trends?

5. Compare to the most recent CHNA to the prior CHNA's if they are available.

6. Summarize the importance of this aspect of population health management. How could

the process be improved? What aspects of the study do you see utilized in planning?
