Suppose you are working as a business analyst and your team just had been hired on a new project - building a hospital admissions system. Below are the details of the case:

The system will handle the admissions process for the Toronto Community Hospital.

Admissions are of two basic types; scheduled and emergency.

To reserve a bed for a future scheduled admission the Admission's clerk will check if the patient already has a patient record by entering the patient's health card no. If the patient does not already exist on file the patient will be assigned a unique patient number and then will patient will be registered with their basic details - name, address, DOB, gender, phone number and next of kin. A reservation record will be created for a bed in the appropriate ward. Reservations have an expected admit and an expected discharge date.

When a patient that has already been scheduled arrives on the appropriate admission date they will present themselves to the Admissions reception. The Admission's clerk will admit the scheduled patient by entering the patient's health card # and verifying with them that their personal data as already registered in the system is unchanged. If it is not they will request the new data and update those details. A bed in the appropriate ward would already have a reservation record for this patient for this expected date. That reservation record will be removed and replaced by an occupancy record for that patient for that bed with an actual admit date and expected discharge date. The status of the bed will be changed from "unoccupied" to "occupied".

When there is an emergency admission, a check will be made for an "unoccupied" bed or one that is not reserved for that date that meets the type required. If there are none this will be notified and the patient will not be admitted. If there is an available bed the new patient will be assigned a unique patient no and registered with as much information as available and that patient assigned to that bed. The status of the bed will be changed from "occupied" to "unoccupied".

When patients are discharged the Admissions clerk will input the patient # and actual discharge date, and the bed's associated occupancy record will be removed and a past admissions record created for that patient. It will contain the actual admit and discharge dates. The status of the bed will be changed from "occupied" to "unoccupied".

In addition to the patient data, data is also retained on the beds and the wards. Each bed in a Ward has a bed # and a room # for that ward as well as an occupancy status. Wards are named, have a specialty designation, a generic bed and are located on a specific floor of the hospital. Obviously, each ward can have many beds.

A report is produced daily showing the current status of all beds for each ward in the hospital.

Patient Services also require access to the system to provide relatives and friends of patients with their ward and room location as well as the ward's standard visiting hours which can be different for each ward.
