This task requires you to manage the incident site of Joshua's fall and gather the necessary information to notify your WHS regulator. STEPS TO TAKE 1. Visit the incident site to coordinate response to the incident with the WHS officers. To do this, instruct the WHS officer to do the following: Clear and preserve the incident site. Notify Joshua's emergency contact about his involvement in an incident Contact the facility in charge of his treatment 2. Consult with at least two personnel who can provide you with information relevant to the accident. 3. Interview each of the identified personnel by collecting the following information: Details of what happened, such as the nature and severity of the incident and its potential cause Persons involved in the incident aside from Joshua Cause of the incident Location of the incident Time of the incident 4. Request for an incident report. This must contain the same information relayed to you from your respondents. The report can be generated by either the WH supervisor or WHS officer.
