Task 3: Workplace debriefing following a work incident Complete a role play which demonstrates your skills in debriefing a staff peer following a hypothetical workplace incident. The staff member and you can decide on a workplace incident that is relevant to that workplace. The incident does not need to be complex or traumatic. You need to demonstrate an understanding of the agency's procedures around this, and skills in the debriefing techniques used in the agency. To show evidence of your ability to complete this task, you need to submit the workplace Supervisor's Statement provided at the end of this section. Please read this and provide your Workplace Superviso with a copy before you complete the debrief session. Task 4: Supervisor's Statement This section should be completed in the workplace. You are required to demonstrate the skills listed below. To show evidence of your ability to complete these tasks, you need to submit the workplace supervisor's statement provided below. A workplace supervisor provides evidence of your skills in the workplace and completes the 'Supervisor's Statement'. The supervisor: 0 Needs to be in a position to make informed comment on your performance 0 Gives consent to be contacted by a TAFE Digital teacher about the process Evidence: The supervisor should identify and comment on a range of evidence of your skills and knowledge. Suitable evidence could include: Direct observation of your skills Questions to demonstrate your knowledge Workplace documents Supervisor report 1. Instructions for supervisor: In the left hand column of the following form are the skills and knowledge a student should be able to demonstrate in order to meet the competencies for each unit. 2. You may comment either directly on these competencies, if observed, or ask the student a prompting question (examples are provided below each element). 3. In the right hand column, please provide evidence that the student does or doesn't meet the competencies through demonstration in the work place, or by answers to the prompt questions
