Jessica is a 25 year old Caucasian female who is selfreferred to youfor substance use and addiction treatment. She reports that she found youragency on psychology today and was interested inreceiving outpatient services. She reported she hasTricareinsurance and comes from a military family. Upon assessing her, shediscloses that she has a severehistory of trauma to include sexual assault and physical abuse, she declines to provide any additional information on this areahowever she notes she has a fear of males and does not like to be "confronted or yelled at."She reportsthat she has been using marijuana dailyup to 4x per day averaging about $10-15 worth each sitting. She has a history of opiates, cocaine andbenzodiazepineuse via snorting whichrequired multiple cycles of residential treatment to enter sobriety. She reports that she has been sober 3 years from these three substances however she has relapsed on marijuana 6months ago and thenrecently relapsed onbenzodiazepinesand opiates for a period of 30 days after discovering that she was 2 months pregnant from an ex-boyfriend. She wouldlike to avoid entering residential treatment and has 1 week of sobriety from thebenzodiazepinesand opiates (fentayl) however she iscurrently experiencing significant cravings,and she continues to smoke marijuana daily.
Update:As you continue to work with Jessica, she discloses that she has no social supports outside of two close friends.Her two friends are also in active addiction and do not encourage Jessica to maintain sobriety.She is currently employed fulltime in customer service and resides with her father, where her insurance is covered. She is not at risk of homelessness however she discloses that herinsurance will expire in a few months on her 26th birthday.She has not sought out medical care for her pregnancy as she is uncertain as to whether she wants to move forward with her pregnancy or explore alternative options. She also has a romantic relationship with Tasha, a female companion who often initiates arguments with Jessica for "cheating" and blames her for the pregnancy. Jessica has a fear of abandonment and often associates herself with individuals who "accept her for who she is." She is open to exploring group counseling, but expressed concerns regarding the dynamics of groups.
