Dear Colleagues and Management,

I am writing to discuss a telephone conversation I had with a client ( Derek), who is eager to come to HOS as a resident. Derek is 40 years old and is currently a resident at the Mental Health Unit at Royal Prince Albert Hospital (RPA). He is suffering from epilepsy and bipolar disorder and is on sodium valproate. He gets a Depo shot once a month and is currently on the Disability Support Pension (DSP) from Centrelink. He has no legal issues.

I would like to invite you to the meeting to discuss thepreliminary assessment of the client (Derek) and if Derek is the right fit to jointhe modality treatment of therapeutic community at HOS.

The meeting will take place on 1/1/2023 from 9am to 9.15 am in the Managers room on the first floor. The agenda for the meeting will include an overview of the telephone interview, a discussion of the client's current situation, and a discussion about how to best support him.

Your input is important in understanding the best way to support Derek, and I invite you to join the conversation.
