Toolkit Competency Application Mini Case Study: Mountain Orthopedics

Jayson is the office director (coordinator) for five office locations that comprise Mountain Orthopedics. This specialty practice sees a wide age range of patientsfrom high school athletes (sports medicine) to senior patients needing joint replacements, and adults with complex spine conditions requiring pain management. While the health outcomes, patient experience, and satisfaction of the younger population are more than satisfactory, the adult and senior populations surgical health outcomes and rehabilitative patient engagement rates are below benchmarks. Jayson was recently introduced to the PHAPHM Framework and feels that organizing the practice to focus on the subpopulation of seniors would provide an opportunity for the health providers, patients, and other health practitioners to achieve higher level quality outcomes.
