The Equifax data breach in 2017 led to hackers accessing sensitive data of 147 million customers in Canada. The sensitive data which included the names, date of birth, home addresses and social security numbers of customers were exploited by hackers' ability to identify a vulnerability in Equifax software. Although Equifax was informed about the possible breach, they had not run a patch to correct the vulnerability in their system. According to this article, the Equifax breach could be an example of misalignment between leadership and cybersecurity governance.
