The following is a summary arising from the information submitted by the onsite supervisor.

Site establishment, set out and excavation was delayed by 2 weeks due to delays on a concurrent project, the unavailability of site accommodation and inclement weather.

Excavation was further delayed because underground services were not properly identified in the set out.

Although the concern was raised about underground services at the toolbox talks in the days prior the individual's given responsibility for confirming locations and marking them did not carry out those tasks effectively. Discussion with the individuals indicates that they did not clearly understand what was required outside of the ordinary policies and procedures.

A land surveyor was required to identify the site boundaries and establish remaining underground service locations.

Additional hire fees were incurred for the excavation equipment and because of the late notice about cancellation additional labour fees for three (3) days were incurred for all onsite workers.
