TREATMENTS (TREATMENT-NO, T-DESCRIPTION, DATE, CHARGE, DRUG-CODE, D-DESCRIPTION, QUANTITY, PATIENT-NO, PATIENT-NAME, BILLING-ADDRESS, DOCTOR-NO, DOCTOR-NAME, SPECIALTY)

Each treatment is prescribed by a particular doctor for a particular patient. A patient can have many treatments, possibly prescribed by different doctors on different days. A doctor can prescribe many treatments, possibly for different patients. Each treatment can have one or more drugs prescribed by the doctor. A given drug can be prescribed more than once, on different treatments.

The TREATMENT-NO field is the unique identifier of treatments. The T-DESCRIPTION field contains the description of the treatment procedure. The DATE field indicates the date on which a particular treatment procedure was prescribed. The CHARGE field indicates the amount charged to a patient for each treatment procedure. The DRUG-CODE field is the unique identifier of the drug used for the treatment. The D-DESCRIPTION field contains the description of the drug, while the QUANTITY field indicates the quantity of the drug to be used on that treatment. The PATIENT-NO field is a unique identifier of patients. As can be seen, the patient's name and billing address are also stored in the table. The DOCTOR-NO field contains the unique identifier of the doctor who prescribed the treatment for a particular patient. As the fields suggest, the DOCTOR-NAME and SPECIALTY fields contain information about the name of the doctor and his/her specialty.
