The healthcare industry has evolved significantly over the past years and one of the main contributing factors to such development is the culture of learning from past errors. Although it is rather unfortunate that some may have had the misfortune of losing their lives of some of these errors, but these errors allowed for permanent change within the system so other patients lives could be preserved. The reporting of errors in the medical field should be encouraged in order to prevent the same errors from repeating. Organization leaders should create a safe trustworthy environment for employees to report errors without the fear of negative consequences. As humans we are expected to make mistakes. However, leaders should ensure that trainings and expectations of employees are made clear and in instances of errors employees feel empowered to report and be open to discussing what led to the error and how it could be prevented in the future. Adopting this culture will increase patient safety, satisfaction and health outcomes
