The patient, Lucy, is a 42-year-old Caucasian woman. She lives in a suburban neighborhood with her husband and two children, ages 12 and 16. Lucy had a somewhat difficult childhood. She tells you she had a hard time making friends and often times her friendships didn't last very long. She speaks very negatively of the people she considered to be her friend. She expresses significant emotional lability and difficulty with emotion regulation. Lucy and her family reported that when she was roughly about 37, she started to become even more bitter and angry at "all the stupid people in the world." She would routinely argue with people telling them what they should or should not do, including strangers. She has even become physically aggressive with people in stores or outside, often yelling at them while recording them on her phone to "show the world how horrible they are." Her family notes that she often yells at home and frequently yelled at the TV, her phone, or her computer. Her husband says she often cries very easy to the point of sobbing and shaking and other times she becomes incredibly anxious to the point of panic. He says she frequently "thinks the worst in every situation." She started to miss work to attend to "important things" and her husband suspected her of having an affair. She denied this, but she does frequently discuss various men that she considers to be "just like me." She tells you she enjoys being around a particular group of people because "they understand what I know." She tells you, "there are women there, and they're 'okay' I guess." She discloses she doesn't like women in general because they are "catty" and "lie or try to steal her man" and she said she's always believed this since she was a teen. Her children are often scared of her mood and they tell you they feel like they are "walking on eggshells" around their mom. They tell you that when she is on her phone or computer, she often becomes enraged and will shriek at them if they attempt to say anything to her. She denies this behavior, but you notice she is becoming more agitated with the negative perceptions of her. Her family tells you she can't handle even the slightest disagreement with her views, so they keep quiet or just agree with her, so they don't suffer any outbursts. Lucy tells you she is a nice, good, and sweet person and she only does what she does because she wants everyone to understand the mistakes they make. Her family does acknowledge she has moments where she is pleasant to be around, but that her mood can change with a single online article or message.
