In either instance, the issue described will be identified as a problem or an effect. Once the existing outcome or effect has been identified, use a fishbone (Ishikawa) diagram or the 5-Whys method to determine the major causes of the organization's problem. The purpose of this assignment is to identify the strategies or process used to validate the data or, in this case, problem.
