  • The supervising attorney has requested you prepar a new Client Intake Questionnaire. Your supervising attorney would like you to research 2 Intake Questionnaires on the internet. Your attorney would like you to then combine the best aspects of each sample located into a new Client Intake Form that can be used by the entire firm. This should be an internal firm document, which would not be seen by clients. At the least, the form should include personal information on the client, their contact information, the manner in which the client will be billed, hourly rate (or contingent fee percentage), type of case, and any important deadlines. You might want to review the information you know you would typically need to insert into Clio for a new client file or conflict check for additional information to include. The time spent on this will be unbilled to any particular client but will be invaluable to the running of the firm.
