Plaintiff Paul White alleges that no one explained the surgery he had in advance, and that he didn't understand the risks. Mr. Alba is a Nurse Practitioner at Banner Hospital. One of Mr. Alba's responsibilities was to educate patients in preparation for surgery or procedures. Since Mr. Silva held his position for 16 years, he is familiar with the procedure followed at Banner for educating patients prior to surgery. As part of this process, Mr. Alba educated patients and reviewed consent forms with patients. Mr. White signed a consent form, but claims that he did so in a rush without understanding.
Mr. Alba's signature is on the informed consent form as the individual who went through the education and consent process with Mr. White. Mr. Alba does this multiple times a day, and does not specifically remember going through the entire process with Mr. White. Banner would like to have Mr. Alba testify concerning what his ordinary discussion would be
