This will continue to focus on the community you have identified in Unit I, which is either the community you live or work in. It is not a fictional community and should not differ from the one previously used.

 

Change is generally driven by need. The difference among most change is how that need is identified and what drives it. For implementation of new service lines, the presence of a need is important. Identifying the need, particularly if it was not a priority already can be hard. Taking a strategic, evidence-based and data-driven approach can help.

 

You will develop a gap-in-service-delivery narrative to present to your agency's leadership in consideration of adding mobile integrated health care in your community. As you develop your narrative, include attributes that are identified as signs of organizational readiness that would support the addition of the mobile integrated health care/community paramedic program.

 

This should be laid out like the example below filling in Requirement field and Components and Sources of Information Field
