Treatment Plan for Tiffany Maxwell

Presenting Issue: Client experiencing loss of family and marriage due to spouse passing away. Client is direct in interactions with others. Has expressed lack of experience with romance while using sex as an act separate from feelings and has expressed strong emotions when confronted with these losses.

The client demonstrated anger when past actions are discussed with a negative portrayal, but is quick to change emotions. Has been using dance as a means to express emotions and feelings in a way they can control and show creatively. Client has demonstrated passion and drive for creating a dance with her dance partner, showing importance for this aspect of their life.

Due to past actions following the loss of her spouse, the client has established and been placed in a role within her community, but has continued with actions to move away from this role.

Diagnosis: For the client, I would say a diagnosis of post-traumatic stress disorder, depression from loss, and borderline personality disorder. The client has demonstrated impulsiveness and difficulty expressing sorrow from loss. I will use Narrative therapy to piece together the client's thoughts as well as follow along the pattern of the client's behaviors following the loss of her spouse.

I will also incorporate aspects of Gestalt as a way to align with the direct nature of the client's presenting personality in an attempt to discuss events leading up to the loss of their spouse. The range of theory utilized as I work to establish relationship will be in hopes to build rapport and determine the most effective actions the counselor can execute to determine and discuss diagnoses.

I want to incorporate the client's passion for dance and use Art Therapy in sessions with the client and express their emotions in a more controlled form as they continue working on their goals with the counselor.

  • Goal: Explore issues relating to sexual impulsive behaviors
    • Client will take note of and document experiences following the death of their spouse and the feelings that arise when thinking about them.
    • Increase time spent discussing the past actions in increments to build up client being able to talk about them without bringing up strong emotions manifesting in a physical space.
  • Goal: Explore relationship with client's family
    • Client will explore relationship with dance and discuss the ways it has impacted her life.
    • Starting with a phone call or dinner, client will begin outreach with friends and family.
