Patient stated, "My goal is establishing the basic foundation of recovery to promote abstinence."

What is one action or behavior that you could start or stop doing over the next 3 months that would increase the chance of you meeting the stated goal?

Objective:Client will: identify in each session 1-2 cessation skills they used since the previous session to cope with high-risk situations.

Intervention:Clinician will: encourage client to identify in each session 1-2 cessation skills she used since the previous session to cope with high-risk situations.

Objective:Client will identify 3 healthy activities they can incorporate into their weekly routine.

Intervention:Clinician will check in with client regularly on her success with incorporating healthy activities.

Objective:Client will list 5 benefits of meeting individual counseling requirements.

Intervention:Clinician will follow up with client's compliance with her attendance to groups and individual counseling sessions.

Objective:Client will identify at least 3 external and 3 internal triggers for heroin use.

Intervention:Clinician will provide education on the difference between external and internal triggers and will provide examples of each.
