Trina is a 32-year-old African American woman who lives in the suburbs of Baltimore with her husband and three children. She reports a strong Christian faith and belongs to her local church. She is currently unemployed. She comes to you voluntarily, and you will work with her on an ongoing basis this semester.

She reports feeling "sad, anxious, and stressed". She and her husband have both been unemployed since the beginning of the Covid pandemic, when the catering business they worked for shut down for good.

Trina's first memories of feeling sad, anxious, and stressed began when she was a teenager, and she would have occasional crying spells. Today, she still feels like she has trouble getting motivated, has difficulty with memory, and overall feels like she has less energy than the average person. Her youngest daughter has trouble sleeping through the night, which means that Trina often loses sleep as well.

She denies any suicidal ideations or thoughts, but occasionally experiences that she calls "panic mode", where she feels so much stress that it feels like her whole body might explode. When this happens, she has a tough time catching her breath and feels a sense of impending doom. This will sometimes last for several hours or several days. This usually happens at the end of the month when it's time to pay all of the bills. Her family complains that she is irritable all the time, and her husband worries that she's not adequately caring for the children.

Trina believes that she was born from a full-term pregnancy and met her developmental milestones at age-appropriate times. She doesn't report any general medical conditions that she's aware of, either in herself or her immediate family. She later added that both of her parents have Diabetes, but remarked that it's "no big deal--everybody has that".

She attended high school, but quit in her senior year. She later pursued the GED after getting married at age 19, but did not complete it. Trina's first daughter was born shortly after her marriage, and her son 20 months later. The youngest daughter is now 18 months old.

Trina has been unemployed for about two years, which she reports is the source of all of her problems. She remarks that "if I could just get a break with a job, this would all be so much easier". She reports feeling guilty and "worthless" as a result of her unemployment, which is compounded by feeling like she can't concentrate on long term goals. She often has headaches, which also cause her to feel like she can't focus. She complains that she has these symptoms nearly every day for several weeks at a time. Then, periodically, "the fog lifts" as she describes it, and things return to normal for a few weeks.

When you talk with her husband, he reiterates that he worries about Trina not taking very good care of the kids. Sometimes, he says, she forgets to prepare meals for the family because she's laying in bed all afternoon. He also worries that Trina no longer wants to be married, because they haven't had sex since the birth of their youngest child almost 2 years ago.

you met Trina and provided a diagnosis in order for her to receive insurance coverage. Later, you examined that diagnosis for some accuracy. Now, it's time to plan out a Treatment Plan for Trina.

Please provide at least THREE long-term goals, each with at least TWO objectives. These can be all for the same issue, or two-three different presenting issues. Please be sure that your goals are all SMART goals. Keep in mind that the feedback most often given on goals when supervisors review treatment plans is that the goal is difficult to measure. Check out the verb in your goal and ask yourself, "how do I measure this?" If the answer is easy, you're in good shape. But, if you find yourself wondering how to easily measure a verb like "to understand" or "to learn", then you might choose a more active verb, like "demonstrate" or "write".
