Part 2. Find a Statutory Durable Power of Attorney form from Texas or your state. Fill out a Statutory Durable Power of Attorney with your personal information naming a person of your choosing as the attorney in fact and naming yourself as the signor. Forms are available from Westlaw Formbuilder, the instructor's library, or various websites. Fill in the blanks and make choices indicated in the document. Submitting a form with blanks is not sufficient! ENSURE all blanks are filled in, even if guessing.
