We would like you to create the post$-$implementation evaluation form that will provide us with user data for the review. The form will assist our team in determining if the new system meets specified requirements, complies with user objectives, and produces the anticipated benefits.

We will hire an independent audit team to actually complete the post$-$implementation evaluation. They will complete the post$-$implementation evaluation as if they are a contractor registered with the site. The evaluation will be completed on our internal intranet using an online form. It must observe good source document and form design as well as layout. It will consist of at least:

ten questions where users evaluate elements on a numerical scale from $0$ to $5,$ inclusive.

a total field that totals the ten question points applied. This field will be read only.

five open$-$ended questions.

one comment section.

Since you do not have access to our company logo$,$ use only our company name, $7$ Bridges Software. The title of the form should be $$Post$-$Implementation Evaluation$.$ We don$$t have a form number. Each evaluator must digitally sign their work and include their phone number and email address. All fields are required except for the Total field and the one comment section. Ensure you include appropriate project and customer information in the Control Zone so we can pair the data with the Contractor$$s Delight project.
