So if we want to assess whether or not we documented well, we could look at patients charts that already, you know, were discharged. We could look at within surveys from the patients on how their experience was. We could watch direct observation of how the services being given. We could also get, together with an individual or focus group and ask questions to seek feedback on experience. And then, once we get this data, we have to analyze it and put it in a meaningful summary that we can take an action on right. What are the most common survey, feedback? What are the common themes of the record review. What did we find out? And what can we do now to correct these performance? Improvement should be built in organizational culture.
