INDICATION: A 63-year-old man with recent left arm and chest pain that occurred at work while carrying a heaving object. He has hypertension and takes a diuretic daily. No other pertinent medical history.

PROCEDURE: He is exercised by Bruce protocol for a duration of 7 minutes, using the treadmill. Maximum heart rate achieved was 135, 86 percent. Maximum predicted was 137. His blood pressure during the stress test was a maximum of 176/84. He denied any arm pain with this exercise. Following exercise, he was placed at rest. At 1 minute, he had STT wave depression and T-wave inversion in the interior and V5 and V6 leads. His blood pressure dropped to 76/40 at 5 minutes. During this time, he remained pain free and felt fine. At the 10-minute mark, the T-wave inversion resolved and the STT waves returned to normal.

IMPRESSION: He was advised that a cardiac catheterization would be appropriate to rule out ischemic heart disease, and this is scheduled.
