Course of care:
OPERATIVE PROCEDURE: Left salpingectomy. SUMMARY: Patient is G3, P0, TA2, at 7 to 8 weeks from her LMP. The patient was seen in the emergency department approximately 1 week earlier with some bleeding and mild abdominal pain. However, the ultrasound suggested a possible early intrauterine gestational sac. The patient returned this morning with the sudden onset of very acute lower abdominal pain followed by faintness and pain in her chest and right shoulder. She was brought into the emergency department by ambulance and was noted to be significantly in pain and hypotensive. She was resuscitated well with fluids and had an ultrasound, which suggested a 6 cm mass in the left adnexa with no intrauterine gestation and lots of fluid in the pelvis and abdomen suggestive of hemoperitoneum. The patient was taken to the operating room urgently. A laparotomy was done and a left salpingectomy had to be done because of complete disruption of the tube and excessive bleeding. Her preoperative hemoglobin was 135, which subsequently dropped to 110 four hours postoperatively and 95 on the first postoperative day. She was not given any blood. Her subsequent hospital stay was uncomplicated and discharge status was good.
