What is your wage at your What do you expect for a starting wage (S/hr)? (Please do not put a range or les Candidates will be considered if wage expectation corresponds with previous work experience/wage and the reception position offered Please provide a clear rationale for your wage expectation. The College of Physicians and Surgeons has 3 Rules for how long a physician's office must keep patient charts after a patient has been seen in the office. There rules dictate when the chart can be shredded. Rule #1 All charts shall be kept for a minimum of 10 years (from the date of the LAST visit) Example - the chart of a patient who is LAST seen on Jan 1, 2010 can be shredded on Jan 2, 2020 (kept for 10 years) Rule #2 Charts of minors shall be kept until the child reaches 20 years of age (independent of the last visit date) Example - The chart of a child born on Jan 1, 2010 (and seen on the same day can be shredded on Jan 2, 2030 (kept till age 20 years) Example - The chart of a child seen on Jan 1, 2010 at the age of 16 years can be shredded on Jan 2, 2014 (kept until age 20 according to Rule #2) Rule #3 Whichever of the above rules results in a longer time (In the 2nd example under rule #2 - the chart would actually have to be kept till Jan 2, 2020 (not 2014) because Rule #1 would result in the longer time) Please answer the following 3 questions relating to the above rules. The questions are NOT related to each other and are independent questions. Your ability to answer the three questions below will demonstrate your ability to process information. Please note that one error is allowed on this test in order to "pass". 1. A child who was born Jan 1, 2009 was last seen on Feb 25, 2010. After what date can the chart be shredded if the child is not ever seen again in clinic after Feb 25, 2010? (Answer in Month/Day/Year format) 2. At what child's age, at the time of the clinic visit, do the two rules (noted above) result in the same length of time that the clinic must keep the chart? years of age 3. On Jan 1, 2011, charts of patients whose birth year is (or prior) can be shredded if rule #1 is satisfied (in other words, the patient was last seen Dec 31, 2000), Provide clear explanations that demonstrate how you got your answers to each question. Simply referring to or restating the rulele) is not considered sufficient (failure to do so will invalidate your answer even if correct)
