Be sure to include, at minimum, the following information: student strengths; presenting problem is identified; frequency, duration, and intensity of behavior is described; consideration of environmental variables; review of the known communication/function of the behavior; target behavior and replacement behaviors are identified; action plan is included; crisis plan is included; and data collection and progress monitoring are discussed.
