When chased by Russian MIG fighter-aircraft, and at a critical point in the chase, pilots would perform a maneuver using the F-105 control-stick, and the result was change in speed of the aircraft from about the speed of sound to zero ground speed!

The maneuver would essentially 'flip' the aircraft and bring it to a stall condition. Due to this extreme maneuver pilots would often black-out. If they had enough altitude, the pilots would recover and live to fight another day, for the MIG fighters would have no idea what happened to the target that had, in prior seconds, been in their sights.

During routine maintenance of the F-105, structural engineers began to find cracks in the upper part of the aircraft just behind the cockpit. Engineers had no idea what caused these cracks.

Problem 2

According to your book, another useful approach for identifying the root cause is the "Five Why" technique (p 190).

1) Perform the "Five Why" technique, exploring potential caused of cracks in the F105 (post below). 1) Compare and contrast your Fishbone diagram and 5 Why analysis (post below).

Problem 3

Your maintenance team believes the problem of the cracks is either g related (g is from Gravitational forces on aircraft)
or from level or maintenance performed, or even the number of flight hours (per week). You do an experiment, counting the number
of cracks (response variable), while assessing the g meter ( "L" does not exceed 4g "H" exceeds 4 g)
, different level of maintenance (Level I is "L", while Level II is "H"), and flight hours
("L" is less than 100 hours weekly, and "H" exceeds 100 hours weekly).

For this experiment assume interaction is not a factor

1) Which factor (g meter exceeding 4, maintenance level, or flight hours) has the greatest impact on number of cracks?
2) What do you suggest?
