Consider three clients who you have supported by working collaboratively with their care network and other potential services using a recovery-oriented practice approach. You may have worked directly with each client or been involved with supporting to facilitate the client's recovery process.

Reflecting on your experience, complete the client template report that follows. Each section in the template provides guidance as to what you must discuss. Provide clear and detailed response to each item.

Remember privacy and confidentiality principles when completing your report. Do not include any information in you report that could identify your client (such as location, their workplace, names of places frequented by the client, etc.

The care network

Describe how you worked collaboratively with the client to identify their care network. How did you build a rapport with the client? How did you work within a recovery-oriented framework? What strategies did you use to identify the members of the care network, the scope of the network, the importance of each person in the network and their current roles? How did you investigate the potential to expand the role of each person in the network or to add people to the network? What steps did you take to identify the potential for members of the care network to positively impact the life of the client?

Describe the strategies that you used to work collaboratively with the client to increase participation in their community of choice. How did you work collaboratively with the client to identify the benefits of community participation? How did you support the client to identify their community (or communities) of choice? How did you provide the client with support and information to uphold their rights, build resilience and to build capacity within their community?

Working collaboratively with other services

How did you work collaboratively with other services and programs to meet the client's needs? How did you establish a network of contacts and service options? Describe the effective working relationships that you established with other service providers and programs. What information did you gather about the other programs including their benefits and limitations? What were the processes that you used to negotiate and document partnerships (refer to relevant organisational policy and procedures)? How did you clarify the scope of each other's roles and work within the defined roles and structures? What strategies did you use to identify the limitations of your own role and/or expertise and to access specialist services to fill any gaps (refer to policy and procedures)?

Monitoring and reviewing service provision

Describe how you monitored and reviewed the services provided by external organisations. How did you ensure that the services provided continued to meet the needs of the client? How did you ensure that the referrals which were in place continued to be suitable and appropriate? What did you do to fill any gaps in service provision? How did you identify where additional services were required? How did you negotiate and advocate for clients to ensure that the programs in place continued to meet their recovery goals? How did you work collaboratively with the client to address any issues that the client was experiencing?

Workplace skills

Describe how you worked safely in your planning and interactions with this client and their care network. Did you communicate with the client about safety? What steps did you take to plan for an emergency/to manage risks and to ensure that you met your duty of care to the client and to members of their care network? How did you monitor and manage your own wellbeing and that of members of the care network? How did you monitor the cultural wellbeing of your client? How did you reflect on your own performance and practice and implement identified improvements? List any relevant organisational policy and procedures.
