I do apologize Hind, however this is not accurate information that you have provided below unfortunately.

You were not working in the OPD on the dates you've noted below. You were not present at the 0730 staff huddles on those dates, you did not sign in on the Attendance sheet for either day and you were not included on the nursing assignment sheet for either of those days. All of these documents can be provided should this be requested by leadership or HR.

Additionally, your Head Nurse, nor the 3 OPD Team Leaders saw you in the clinics on either day.

We have not seen you physically working in the OPD since your last shift on Thursday May 9^th, which can be verified by the attendance sheets.

Our understanding was that from the period of Sunday May 12^th onwards, you were working in your new role in PNU- Research.

Can you please meet with myself and Fabiana this week so that we can discuss this further and review your attendance so that we can get this sorted
