  • The organisations commitment to protecting client privacy and personal information.
  • WHAT information (type) is collected
  • WHY the organisation needs to collect personal client information.

ROLES/RESPONSIBILITIES

Explains who Policy covers and their roles/responsibilities.

(e.g. Ensure systems in organisation protect information)

Managers

(e.g. Ensure employees are aware of record keeping responsibilities & Policy)

All employees, including volunteers

(e.g. Capture & secure records, Comply with Policy)

PROCEDURES:

  • How can organisation collect information e.g. from person directly/ referral, individual consent?
  • Security and Protection of records (e.g. How the organisation stores securely from unauthorised access, both electronically and paper-based).
  • Disclosure of personal information e.g. Referral, informed consent, health situation exists
  • When records disposed/destroyed
  • And whether this information will be released (disclosed) to any other 3rd party (and in what circumstances).
  • Access to records
  • Client access to records e.g. with Freedom of Information Act 1992
