-You are the quality manager in a small organization. Many pieces that you are producing are defective. Your company is losing market because of this problem, there are plenty of devolutions everyday. The employees are complaining about the overtime because they have to receive the defective pieces, repacking, write their codes and storage them again. The aditional time is not being paid. Many employees want to quit and it is affecting the work environement.

The maintenance of the equipment (which produces the pieces) is out of date because it is a new adquisition and the Directors consider that there is no need to invest in that yet. Nevertheless,the cost of receive the deffective pieces, the customers complaints (many of them are not coming back), the products changes and devolutions, the work environment is getting worse, the cost to hire new people, and many other problems are more expensive than the maintenance of equipment.
