As per requirement from the Department of Environment (DOE), chemical wastewater from your plant needs to be treated before being released into the nearby river. You are engineer in-charge of the wastewater treatment system within the plant. During the routine check and maintenance of the wastewater treatment system, you noticed that there is an intermittent problem with the system which causes the treatment process to halt for a while. Hence, there is a potential of a slight leakage of chemical wastewater flow from the treatment system into the river. The leak into the river could be causing fish to die and some illness in the local community which makes the situation more serious. Therefore, you report to your Plant Manager (Mr. Ahmad) and suggesting a few solutions to the problem. Surprisingly, Mr. Ahmad rejects your proposals because the cost of the proposed rectification work is too high, and the problem only occurs intermittently and the amount of wastewater leak into the river is very minimal. Mr. Ahmad also warns you not to disclose the issue to the DOE or your job is at stake.
