you have been put in charge of developing an incident tracking database for a regional airline. Your database must consist of a minimum of 5 tables but no more than 8 tables, with sufficient fields to store at least the following data: 

• General details of the incident and investigation (Date, who submitted the report, Aircraft, Narrative, Date, Root cause, Investigation Details etc) 

• Aircraft details in a separate Table (Aircraft Type, Registration etc) 

• Employee details in a separate Table (Name, Date of Hire, etc)

 • Incident Category (Name, Brief Description) Data Tables are to be properly related to each other utilizing the Lookup Wizard. In addition the data tables, you must have the following GUI forms in your MS Access Database: 

• A Basic Menu

 • Entry Form

 • Browse Report Form

 • Existing Report Form (including Investigation Fields) 

• At least 2 Reports (of your choice)

 Your final package will include the following:

• ER Diagram (Visio or Hand drawn/scanned) 

• Database model Diagram (Visio) 

• A Concept diagram of the Incident Entry form only
